LOD bug aboose

I was arc warden and had arctic burn, blink, lunar blessing, poison sting, focus fire and impetus.
At some point at min 40, i died to aa+zeus ulti while i was in wyvern form. When i respawned, I was in perma wyvern form still lol. I died again and respawned again in wyvern form. When I killed undying around min 50, I clicked arctic burn again and i transformed to undying LUL. Anyone seen something like this before? @Burnt ???

This game too. Pink was TC, but permanently became Dirge at min 29.
