Violated Rule(s): griefing and feeding for the potm, and refuse to kick for the enemy team.
Time of Violation (in-game or replay): since 11 minute to the end of game, this potm never went with team, alone pushing and complaining about teammates. And he always died alone or run into five after all the teammates death.
Any further thoughts:

Confirmed on the griefing, refusal to team play. Mainly just farmed lanes the entire game while periodically afk'ing at the fountain. Insulted most of the round, didn't do much for the team at all. Kept running into enemy ganks.

No one on the opposing attempted to votekick in this process.

subzero@europe banned 5 days for refusal to teamplay / defend.

However, not banning anyone for not vote kicking as reason was not explained until 20 seconds left in the last vote kick against him
