Why you should be unbanned: Hey guys, checked the ban request for me and I give the credit to the guy that reported it. He was very thorough and I did AFK at the end of the game. I don't have any good excuse for it and I'll own up to my mistake. I apologize to my teammates in that game for AFK and I'm sorry for breaking the rule. Will appreciate it a great deal if someone can unban me this one time? Cheers everyone

Do not go away from keyboard for an excessive amount of time, especially without telling other players beforehand, and/or refuse to play the game in an attempt to ruin it. Neither are you allowed to grief by resetting the afk timer.

Is there any particular reason you play only 1-2 games per account, and then create a new one and repeat?
I am only saying this, because the penalty you face when it comes to breaking the rules in an account with only 1 game, is much more severe than in an account with many games.
