Bot says I was banned by albi for game ruining but I honestly don't remember a game where I refused to play. I tried to look up a ban request/processed ban request, but couldn't find anything under my name.

There was this ONE game like a couple weeks ago where my team was feeding so me and some of my teammates voted to forfeit, but didn't get enough votes so I hugged fountain. But even so, I wasn't afk and still came out to fight.

Can I at least know what game it was that caused me to get banned for 5 days?

That is my brother's account. He visits me often and plays on his laptop so I'm guessing this is Range ban?

Sun US.EAST & Sexy US.WEST are the only accounts that we have used at this ip address.

Both in the same game, don't seem to be same person. This will help you get this sorted out, when a moderator gets to this thread.
But keep in mind, shared connection = shared responsibility, in case he broke the rules while at your place. Otherwise your IP is in his history and any account affiliated to that IP gets banned.
You will probably be asked to not let your brother play while he is banned and so on.

Thank you Hutzu!!! I will try to prohibit him from playing but I work in the evening so I can't prevent him from playing at my house if I'm not home. I hope the moderators can find a way to just give access to my account and not his since he doesn't know my password.

If that's true, then a temporary whitelist of your account will do the trick. If they unban the IP, then he could just create a new account and keep playing.

As @Hutzu said shared ip=shared responsibility, now i will unban your ip and your nicknames obviously under one condition. You talk with your brother and sort this out. There won't be a whitelist in this case for obvious reasons.
If you can do this for me I will unban you right away

Albi, thank you so much! I have already contacted my brother and told him he is forbidden to play for 5 days. Please let me know if you there is anything else you need me to do.

Hope this won't happen again. Unbanned
