please link the general rules and the game specific rules of the game(s) you got banned in, and highlight the specific rule(s) you believe you violated.
You can find them on the Wiki.

That way, ENT Moderators can ensure another rule-violation by you, caused by not-knowing the rules, will not happen again. After linking to the rules, they will expect you to have read them. Cooperating with them will have a positive effect on your ban duration.

push.that.lane was afk for 13 minutes refusing to pick spells and at the fountain. 0:00-9:15, 12:05 - 16:20
He intentionally fed mid purposely 2 times. it was not due to the fact that he was a low skilled player.
3 votekick were intiated and you did not accept to kick him.

Try reviewing the replay before making a statement that you were banned unfairly. It's clear you know the rules and you've been a long time player, So I don't think you need to review the rules because you did not know them.
