Any further thoughts: Azteca complained that we stole kills even though I had more assists than him. He then goes to farm and refuses to co-operate with the team even when we asked him for help. He even admits to throwing the game

After reviewing (replay timers):

[*] Until 23 minutes, everything was okay - At this time, bara gets mad for dying, blaming the team for taking the kills, and thats when things went down the pipe
[*] 28 minutes - sentinel pushing bot, bara is mid farming, and doens't go help bot (sk was farming woods too at this time), only 2 attempted to defend bot -> 1st tower goes down
[*]29 minutes - sentinel keeps pushing towards the 2nd tower, engaging scourge heroes, bara is still mid (sk still farming woods) - bara only comes at 29.30
[*]31 minutes - two instances actually

[*]Right before the mid fight (around 30 seconds earlier), puck and sk attempted to kill them at ancients (sentinel side), bara never charged
[*]Mid fight, bara waited (when he could have charged a lot earlier)

[*]35 minutes - scourge pushing mid, bara farming ancients (scourge side) -> eventually sentinel comes mid, teamfight occurs and bara charges in late
[*]39 minutes - puck even pings top, to gank & push - Bara ignores, and charges bot
[*]39.30 minutes - scourge tp's bot where bara is pushing and where is bara? he goes back obviously! xD (bara went farming woods, when his team tried to push, and dind't try to assist them, when they were running)
[*]41/42 minutes - Team asks to push mid, yet bara charges in late again
[*]45 minutes - scourge pushing bot, bara is top (farming/pushing)
[*]46 minutes - bara goes back (running from bristle), and once he's safe, instead of going help bottom lane where his team was pushing, he just backs out

After he got warned by Beerlord ingame that he could be potentially banned had he continued, he stops it, and starts helping/playing/following his team

azteca@uswest.battle.net should be banned for refusal to play/help his team (not the first time this happens either viewtopic.php?f=24&t=62619 / viewtopic.php?f=24&t=48910 - Old bans indeed, but still not the first time ^^)
