Time of Violation (in-game or replay): In the first 20 min he refuses to help team - there isnt one timestamp.
Any further thoughts:

We asked him several times, but he refuses to leave mid to help lanes. We had to tele around to try and hold lanes, but we could not since we had a leaver from start. So in essence we played 3v5 since he refused to help.

We lost both top lane and both lane because he would not leave mid to def, even just for 2 min. Just for one gang. But nope.

This lasted 20 min. and by then the other team was so fed that we could not move around and the game was in essence over.

Since he is a teamkiller i initiated a legit votekick, where he said he would try to get me banned for is. But this was LEGIT

Since it is not often banned for people who wont help team, and often they need to be "gone" or solo longer, the RULES DOES SAY that refusing to help or cooperate with team is not allowed.We asked several times and he easily had 10-15 min to react, but he would not.

Thus after 15-20 min the game was in essence over, team had given up and wanted to rr or to stay inactive. We did try several times by then they where to high lvl and too fed.

The player ignored you after you called him a moron. There was no refusal to help team. The entire team got beat pretty soundly, and you certainly had a role in that - at the time of the votekick you were level 5 while the rest of your team was 8-9. There was no "we" in the "asked several times", there was only you, with your typical toxic communication style.
