Rule player violated: mirr3 used bear form and reactive armor glitch which could've crashed game/pheonixx used windrunner ult and zeus passive glitch which gave him perma fast attack.
Time of incidents (Note in-game timer or replay timer): Beginning of game, team warned them of glitches when we saw them picking skills then throughout game when they took advantage of the glitches.
Any further thoughts: Trainer76 tried using votekick but failed to pass it. At first I didn't want to kick phoenixx because then I figured we'd have to kick mirr3 and play 3v5 which I hate, but after a bit trainer76 convinced me to !yes the votekick, but I had done so just after it had expired. Also, it turned out the Sent team wasn't too great, so phoenixx took full advantage of the glitch. After being asked not to use bear form, mirr3 still used it and when he died, he lagged the game for a little bit.

(Although he did not crash the game, he did utilized True Form. Thus he is banned regardless, his skill set caused Sentinels to refrain from attacking/killing him)
